What does Tirzepatide muscle loss mean for your strength?
SURMOUNT-1 body scans found loss of fat and tissue that was not fat, including muscle. The studies tell us much less about strength and movement.

Does weight loss with Tirzepatide include muscle? Yes. Most large weight loss includes some muscle and other tissue apart from fat. The top-dose average weight loss was about 20% over 72 weeks. A large, fast loss can mean more pounds of non-fat tissue lost.
In the SURMOUNT-1 body-scan study, tissue besides fat made up about 25% of lost weight. Fat made up 75% [6]. A 2026 review put the middle non-fat share near 28% across similar drugs [26]. A 2024 review compared the amount with what may be lost through at least a decade of aging. Weight training may help protect muscle [27].
Tirzepatide muscle loss is real. It doesn't appear worse than muscle loss seen with other large, fast weight changes. The effect on your strength and movement is still unknown.
What SURMOUNT-1 Tirzepatide muscle loss scans found
The clearest figures come from a SURMOUNT-1 side study using DXA. This is an X-ray scan that separates body fat from tissue that is not fat. It can't count muscle by itself.
After 72 weeks, people taking Tirzepatide 15 mg had lost 20.9% on average from their starting weight [1]. Tissue besides fat made up about 25% of the loss. Fat made up 75% [6]. For a person losing 20 kg, those shares would equal about 5 kg of non-fat tissue and 15 kg of fat. That is only an example from the group average, not a promise about your body.
A 2026 review joined 24 studies with 2,262 people. Across similar drugs, the middle non-fat share was about 28%. Tirzepatide was near the middle [26].
A 2024 review compared the amount lost with a decade or more of aging. Small early studies suggest weight training may reduce the loss. Large studies haven't settled that question [27].

What body scans cannot tell you about strength
Do these scan changes mean you will feel weaker? The main studies don't give a firm answer. Strength, walking, and how fast your body burns energy need more study.

A 2025 study of older adults with type 2 diabetes found that low blood sugar could be managed with their other care [23]. Muscle strength was not a main question.
What weight training and protein may do for muscle
Weight training and enough protein are discussed most often. Proof for people taking Tirzepatide is still limited.
A 2024 review asked whether weight training can protect tissue besides fat during weight loss [27]. Small early studies found less loss when people followed a set training plan. Few large studies have tested the drug and training together. The review found a sound reason for more study, not final proof.
Protein can help protect muscle when people eat fewer calories. The Tirzepatide studies did not give everyone the same amount. They can't tell us how much protein changed muscle loss during treatment.
